Requirements

  1. Application forms;
    • A. Principal Borrower
    • B. Co-Borrower, if any
    • 2 Copies each, back to back on a Long Bond Paper
    • Write all entries in CAPITAL LETTER, Avoid having erasures
    • Two (2) 1x1 pictures of each borrower (white background only)
    • Don't write "same as above" with the details that have the same information, write completely and legibly.
  2. Copy of 2 valid IDs (affix 3 original signatures)
  3. Birth certificate/Marriage contract (whichever is applicable)
  4. Letter of consent (Authorization letter to conduct employment verification) for locally employed and seafarers only.

For Self Employed, any of the following Proof/s of Income:
  1. ITR, Audited Financial Statements, and Official Receipt of tax payment from bank supported with DTI Registration and Mayor’s Permit/Business Permit
  2. Commission Vouchers reflecting the issuer’s name and contact details (for the last 12 months)
  3. Bank Statements or passbook for the last 12 months (in case income is sourced from foreign remittances, pensions, etc.)
  4. Copy of Lease Contract and Tax Declaration (if income is derived from rental payments)
  5. Certified True Copy of Transport Franchise issued by appropriate government agency (LGU for tricycles, LTFRB for other PUVs)
  6. Certificate of Engagement issued by the owner of a business.
For Overseas Filipino Workers (OFW):
  1. Employment Contract § Employment Contract between employee and employer, § POEA Standard Contract.
  2. Certificate of Employment and Compensation (CEC) § CEC written on the Employer/Company’s official letterhead; or § CEC signed by employer (for household staff and similarly situated employees) supported by a photocopy of the employer’s ID or passport.
  3. Income Tax Return filed with Host Country/Government NOTE: If documents are in foreign language/s, English translation is required.
Additional Reminders:
  • Kindly send a soft copy first to Account Officer, before sending the original copy of the documents.
  • Additional requirements will be informed once the account is done with the pre-assessment.
